Title:Hepatic morphology in reactional states of leprosy.

Abstract:Liver function tests and liver biopsies were studied in 23 leprosy patients in reaction and 10 without reaction. The liver biopsies in leprosy patients with reaction showed exudative lesions, epithelioid and tuberculoid granulomas, and foam-cell granulomas. Portal vasculitis was encountered in a few cases. Neutrophilic infiltration into the foam-cell granulomas was seen in a few cases of lepromatous (LL) leprosy with reaction. In six cases of borderline (BL, BB and BT) leprosy with reaction, a spectrum of lesions bearing footprints of exudative lesions were seen evolving into epithelioid-cell granulomas. Foam-cell granulomas and tuberculoid and epithelioid granulomas along with exudative lesions were encountered in two cases on individual biopsy strips. An altered albumin-to-globulin ratio was the chief functional derangement observed in these cases. The spectrum of changes observed in borderline leprosy with reaction could be discrete steps in the evolution of upgrading reaction^ien.

Abstract:The prevalence of conjugal leprosy was studied in the records of the dispensary of Campinas (S.Paulo) from 1934 to 1954, showing 50 mates among 500 contaminated contacts. This is 7,8% of 639 mates under control to be compared with 8,8% of contaminations among 1.365 brethren, 9,9% among 2.064 children and 9% among 420 parents. 30% of the mates have been examined regularly (at least once yearly) against 16%, 23% and 25% of brethren, children and parents respectively. Of the 50 contaminated mates 52% were males. Among the wives there were 41% L and 46% T cases, against 37% and 44% among husbands. Duration of marriage was up to 10 years in 44%, incubation period up to 5 years in aproximately 94%.(AU)^ien.

Title:Hanseníase tuberculóide da variedade nodular da infância.

Abstract:Estudam caso de hansen¡ase tuberculóide da variedade nodular da infância (Souza Campos), observada em criança de 2 anos e 7 meses, do sexo feminino, branca e que desde o nascimento mantinha contato com o pai, portador da moléstia do tipo V, com baciloscopia fortemente positiva (+++) em L.C. e M.N. Apresentava 4 nódulos localizados no membro superior esquerdo, sendo o maior de 11 X 10 mm e os demais de 4 X 5 mm. Eram duros, indolores, aderentes aos planos adjacentes, de superf¡cie lisa, de tonalidade eritematosa e, quando distendidos, tornavam-se translúcidos e de cor branco-amarelada. O exame histopatológico revelou granuloma tuberculóide (tipo sarcóide) com ausência de b.a.a.r., os quais também foram negativos em baciloscopia de L.C. e M.N.. A reação de Mitsudina foi fortemente positiva (nódulo ulcerado de 8 X 11 mm). Na evolução, as lesões mantiveram-se estacionadas nos primeiros 17 dias; do 35º ao 70º dia houve aumento de tamanho e a partir do 87º dia iniciou-se a regressão. No 125º dia de observação a redução era de 50%. Discutem a influência do fator idade na incidência da moléstia e apresentam os argumentos que justificam a individualização dessa variedade de hanseníase. (AU)^ipt.

Title:O comprometimento neurológico na lepra.

Abstract:A ação potogenética do Mycobacterium leprae se deve à invasão gradual do tecido conjuntivo, pelo qual tem marcada afinidade, e à reação extremamente variável em tipo, intensidade e extensão, que o organismo opõe àquela invasão. Do conjuntivo da derme, que se presume local habitual de implantação, o germe principia sua lenta marcha invasora, atingindo as estruturas nervosas cont¡guas, isto é, as terminações e ramúsculos periféricos. Destas lesões iniciais, resultam as primeiras manifestações cl¡nicas, tais como alterações dos reflexos sudoral, vasomotor, distúrbios das sensibilidades térmica, dolorosa e táctil, hipotricose e hipopigmentação. Máculas hipocrômicas ou eritêmato-hipocrômicas, acompanhadas de anidrose, alopecia e arreflexia vasomotora são muitas vêzes observadas; contudo, as lesões ramusculares podem desenvolver-se com o m¡nimo, ou mesmo ausência, de alterações apreciáveis para o dermatologista. As lesões invadem gradualmente as terminações e ramúsculos nervosos periféricos, propagando-se às áreas vizinhas, assim como ao tecido conjuntivo dos troncos nervosos (neurite ascendente). Por outro lado, a possibilidade de desenvolvimento, por via hematogênica, de focos inflamátorios metastáticos nos nervos é aceita por muitos pesquisadores. Assim, as neurites de tipo ramuscular ou troncular, representam, essencialmente, o substrato anátomo-patológico da neuroleprose. Do ponto de vista, histológico, observa-se de in¡cio, geralmente, uma estrutura inflamatória simples, linfo-histocitária, que poder  evolver no sentido de uma estrutura lepromatosa, tuberculóide ou dimorfa, conforme a reação oposta pelo organismo. Episódios reacionais agudos e subagudos podem acarretar agravamento súbito das perturbações neurológicas preexistentes ou mesmo promover o seu aparecimento. A classificação atualmente aceita das "formas" da moléstia adota como fundamentais o grupo Indeterminado (I), o tipo Lepromatoso (L), o tipo Tuberculóide (T) e o grupo transicional, Dimorfo (D)...(AU)^ipt.

Abstract:(Role of temperature, microtraumatisms by elongation or subluxation and canalar stricture.) Cold has a slight aggravating effect on leprosy which is observed in some countries as Iran, South Russia, North India and the Andean cordillera. The subluxation of the ulnar nerve has facilitating effect in the development of the neuritic damage in a limited number of patients. The elongation is important for the production of the ulnar neuritis. The canalar stricture is a major factor causing nerve damage in leprosy. It explains the segmentary localization of the principal damages of the leprous nerves in the proximal areas situated above the tunnels. Intraneural hypertension appears first and is responsible for the swelling of the nerve which gets entraped inside the tunnel. A circulus viciosus is, then, created which maintains and aggravates the intraneural hypertension.

Abstract:A brief review of the contradictory opinions of different authors as to the nature and the place of tuberculoid cases of leprosy in the evolution and pathogenesis of the disease is presented. Mention is also made of their failure to agree on what could be generally considered as the esential histological features of this condition. A case of tuberculoid leprosy which has undergone transformation into a lepromatous-type case within six years of observation is presented. This case report together with the clinical, bacteriological and histological studies of other authors seem to agree with the opinion of Manalang that tuberculoid leprosy is a stage in the evolution of leprosy, not immutable as assert by Schujman. That such transformation among adult tuberculoid cases is rare because they represent the filtered resistant ones or benign infections, is indicated. The need for continued follow-up and life-long study of contact children born of leper parents in institutions is indispensable if we are to know the evolution of leprosy. (AU).

Abstract:Rheumatic manifestations are a common feature of leprosy. The present study was performed to assess the incidence of rheumatic manifestations in leprosy. Methods: seventy cases of leprosy attending the indoor and outdoor departments were studied. Results: Out of the 70 cases studied, rheumatic manifestations were seen in 61.42% of cases: arthritis in 54.28% and soft tissue rheumatism in 17.14%. Enthesitis was seen in 2.84% of cases. The incidence of arthritis in lepra reactions was 57.57%. Conclusions: Musculoskeletal manifestations can occur at any time during the infection. Articular inflammation in leprosy usually occurs in reactive states, particularly erythema nodosum leprosum (ENL). Leprosy can present with arthritis as the first symptom.

Abstract:The reactional states seen in lepromatous form (erythema nodosum, leprosum, exacerbation) and i interpolar forms (tuberculoid exacerbation and tuberculoid reversal reaction) result in cutaneos, nervous, lymphatic, visceral and polyarthritic acute manifestations. The sucessif relapses result in articular, cutaneous and muscular sequelae of the hand but a paralysis of medial and ulnar nerves, often associated with leprosy, make them different from rheumatoid arthritis. They can be summarized thus: 1- Fixed claws hand associated with stiffness, erosions and sometimes subluxations of interphalangeal joints, mainly proximal and metacarpophalangeal joints...AU.

Abstract:The question of the prospect for absolute cure, oron the other hand that of transformation to the lepromatous type, of active major tuberculoid cases, especially those that at one time or another have been found bacteriologically positive, is discussed. Out of twelve such cases observed at Cebu for several years, no less than seven have relapsed at one time or another after apparently complete recession of the disease. In one instance there had been a period of five years without trouble. It is suggested that the feature of relapse in tuberculoid leprosy is worthy of more attention than it has received. Lepromatous transformations has not as yet occurred in any of these cases. Two of them, however, have persisted for three years and more in an unstable, borderline condition, the picture dominated by repeated papular eruptions, the lesions atypical clinically and histologically. Another borderline case of decidelly different aspect seen in China is cited; it presented at the same time apparently lepromatous, tuberculoid and intermediate lesions and was looked upon as transitional, yet later recession occurred as is common in ordinary tuberculoid leprosy...AU.
